Three Mistakes We Made

Mistake 1: Information Silos

Each agent has separate workspace and memory. Fix: Write all decisions to files. Files are the Single Source of Truth.

Mistake 2: Overlapping Responsibilities

Two agents handling the same task. Fix: SOUL.md defines clear boundaries, one task = one assignee, PRD is the single task source.

Mistake 3: Duplicate Reports

Multiple sub-agents complete simultaneously. Fix: messages.queue.mode=collect, 2s debounce, maxConcurrent=1.

Core principle: Text > Brain. Anything not written down is lost on session restart.
